What Is Acne?

Acne vulgaris is a chronic inflammatory skin condition that occurs when hair follicles become clogged with oil (sebum) and dead skin cells. This creates an environment where Cutibacterium acnes bacteria thrive, triggering inflammation and the characteristic lesions most people recognize as pimples.

Though often thought of as a teenage condition, acne affects people of all ages. According to the American Academy of Dermatology (AAD), 85% of people between the ages of 12 and 24 experience at least minor acne — but adult acne is increasingly common, particularly in women over 25.

Types of Acne

Acne exists on a spectrum from mild to severe, and identifying the type helps determine the right treatment:

  • Whiteheads (closed comedones): Pores clogged and sealed beneath the skin surface.
  • Blackheads (open comedones): Clogged pores open to the surface; the dark color is oxidized sebum, not dirt.
  • Papules: Small, raised, red bumps caused by inflamed or infected hair follicles.
  • Pustules: Papules with visible pus at the tip — what most people call a pimple.
  • Nodules: Large, solid, painful lumps deep in the skin. Prone to scarring.

What Causes Acne?

Multiple factors contribute to acne formation:

  • Excess sebum (oil) production driven by androgens
  • Follicular hyperkeratinization (dead skin cells that don't shed properly)
  • Overgrowth of Cutibacterium acnes bacteria
  • Inflammation triggered by the immune response
  • Hormonal fluctuations (puberty, menstrual cycles, pregnancy, PCOS)
  • Stress, which elevates cortisol and androgen levels
  • Certain medications (corticosteroids, lithium, some birth control)

Is acne only a teenage problem?

No. While acne is most common during adolescence, it affects people of all ages. Adult acne — particularly in women — is increasingly common and is often driven by hormonal fluctuations, stress, and certain skincare or hair products. Up to 15% of adult women experience persistent acne.

What causes adult acne?

Adult acne is most commonly triggered by hormonal changes (menstrual cycles, pregnancy, perimenopause, or conditions like PCOS), stress-related cortisol spikes, certain medications (corticosteroids, lithium), comedogenic skincare products, and in some cases, diet (high-glycemic foods and dairy may contribute in certain individuals).

How long does acne treatment take to work?

Most topical treatments take 6–8 weeks before significant improvement is visible. Oral medications may show results faster. Isotretinoin (Accutane) typically produces dramatic results over a 4–6 month course. Patience is important — changing treatments too quickly can slow progress.

Can acne cause permanent scars?

Yes. Cystic and nodular acne carry the highest risk of scarring. Picking or squeezing blemishes significantly increases that risk. Treating acne early and aggressively — before lesions become deeply inflamed — is the best way to prevent permanent scarring.

What is isotretinoin and is it right for me?

Isotretinoin (brand name Accutane) is an oral retinoid and the most effective treatment for severe, scarring, or treatment-resistant acne. It works by dramatically reducing oil production, unclogging pores, and reducing bacteria. It requires monthly monitoring and pregnancy prevention measures due to serious birth defect risks. Your dermatologist will evaluate whether it's appropriate for your case.

Does diet affect acne?

Research suggests that high-glycemic diets (white bread, sugary drinks, processed foods) and, in some individuals, dairy consumption may worsen acne. However, diet is rarely the sole cause.
